You can use the third-party service, Wufoo, to create a customized form for your site.
Note: To receive troubleshooting support for form issues, we suggest using the Form Block, a built-in way to create customizable forms. Wufoo is an alternative for forms that require an option to upload files or images.
The solution in this guide includes using a Code Block to add advanced code. This is a Premium feature available in the current Website Business plan and higher:
- Available in: All Business and Commerce plans
- Available in: Legacy Personal plan (no longer available for new subscriptions)
- Not available in: Current Personal plan
- Trial: You can use the Code Block during a trial.

Step 1 - Create a Wufoo form
Create a form in your Wufoo account.
Note: File storage is only allowed on paid Wufoo accounts. To learn more, visit Wufoo’s pricing page.
Step 2 - Create the Wufoo code
In the Wufoo Form Manager, click Share in the menu for the form you want to embed.
Scroll down to Advanced Sharing Methods and copy the iframe Embed Code.
Step 3 - Add the Wufoo code

Paste the Wufoo code into the Code box. Click Apply.
