You can collect information from visitors with a Form Block, Custom product form, or a Custom checkout form. While these forms have different uses, they have the same 19 form fields that you can use as the building blocks for your form.
Note: Forms connected to Mailchimp can have up to 30 fields. We recommend this limit as a general rule to keep a form user friendly and quick to load.

Depending on where you added your Form, the Form may have some default form fields. You can edit these or remove them.
Use the Text field to collect basic information from visitors. Only one line of text can be entered here, so this field is best if you're expecting a short reply.
For longer replies and a larger text field, add the Text Area field.
The Text Area field doesn't have a character limit.
You can create a drop-down menu with the Select field.
Create a title for the drop-down and list each option on its own line. Visitors can select one option.
You can create a list of multiple options with the Checkbox field.

To format the options, list each option on its own line.
This is similar to the Select field, except with a different display style.
You can add multiple options. For each option, the visitor rates on a five-step scale from "Strongly Disagree" to "Strongly Agree."
To format the survey, list each option on its own line.
In the form submission email, survey answers are displayed using a numerical scale from -2 to 2. "-2" will represent "Strongly Disagree" and "2" will represent "Strongly Agree."
The Name field requests the visitor's first and last names.
The visitor can enter hidden text that will be visible upon receiving the form submission.
This doesn't create a password for logging into the site; it's data that is recorded with the submission for the site owner to collect.
Several text fields will be presented to collect a visitor's address.
Note: The address fields will be formatted for US addresses, and they can't be changed for a specific international format.
This form field will display an @ symbol so that the visitor can add their Twitter handle.
This field prompts the visitor to enter their email address. It must be in the proper format to be accepted.
Note: The email address email@example.com won't be accepted.
The Website field will show http:// in the field so the visitor can enter a website.
Using the Date field, you can ask the visitor to input a date into formatted fields.
Note: The date format can't be changed. It will always be Month/Day/Year.
In the Time field, a visitor can enter a time, which will be submitted in a time format.
Note: The format can't be changed to the 24-hour clock. It will always be AM/PM.
This field adds areas in which the visitor can input their telephone number.
If you check Show Country Code, a fourth field will display to collect the visitor's international phone code.
Note: The phone number fields will always be formatted for US telephone numbers. They can't be changed to an international format. As an alternative, use the Text field instead.
Only numbers are accepted in the Number field.
In the Currency Field, a user can enter an amount of money in front of a currency symbol of your choosing.
Note: You can change the currency symbol to something other than USD.
The Line field adds a horizontal line, which can be useful for adding a visual break to longer forms.
You can replace the Section text to title this section of the form, and describe the section in the Description text area. Uncheck the Underline option to create a text-only break.
